The progression of and optimal surveillance intervals for branch-duct IPMNs (BD-IPMN) has not been widely studied. We evaluated the progression of BD-IPMN Mucinous cystadenomas (MCN) of the pancreas are a type of mucinous cystic neoplasm of the pancreas. Epidemiology Previously believed to occur exclusively in middle age females 5, it has occasionally been described in males 6,7. Pathology It i Incidental Cysts in the Pancreas • Incidental cysts of the pancreas are common • More likely to be neoplastic rather than benign –30% of all incidental masses were IPMN in one series –55% were MCN or IPMN (preinvasive precursors), only 4% were pseudocyst • Pathologist is reviewing the cyst fluid to Pathology, Teine-Keijinkai Hospital, Sapporo, noma may arise in the pancreas with IPMN in either an associated/derived way or a distinct/concomitant way,1 and Microscopy of pancreatic mucinous neoplasia with IPMN with early invasion.

IPMN tumors produce mucus, and this mucus can form pancreatic cysts. Although intraductal papillary mucinous neoplasms are benign tumors, they can progress to pancreatic cancer. pancreatic masses as benign or malignant [25, 26]. MD-IPMNs are more accurately diagnosed preoperatively than BD-IPMNs, however CT imaging can often identify side-branching pancreatic duct cysts seen in BD-IPMN and mixed type (Figure 3) [27].

On sectioning, the main pancreatic duct or one of the branches is usually dilated. The duct is often filled with mucin and is tortous and irregular.

Maitra A, Fukushima N, Takaori K, Hruban RH. Precursors to invasive pancreatic cancer. Adv Anat Pathol. 2005 Mar;12(2):81-91. Pancreatic intraductal papillary mucinous neoplasms (IPMNs) rank among the most common cystic tumors of the pancreas. For a long time they were misdiagnosed as mucinous cystadenocarcinoma, ductal adenocarcinoma in situ, or chronic pancreatitis. CEA and CA 72.4 in pancreatic cyst fluid have excellent NPVs in the preoperative differential diagnosis of benign versus malignant IPMN, and might reinforce the decision of not to operate on patients with BD-type without predictive factors of malignancy.

The duct is often filled with mucin and is tortous and irregular. Intraductal papillary mucinous neoplasms (IPMNs) of the pancreas are potentially malignant intraductal epithelial neoplasms that are grossly visible (typically >10 mm) and are composed of mucin-producing columnar cells. However, depending on the morphological (MD-IPMN, BD-IPMN) and histological subtype (intestinal, pancreatobiliary, oncocytic or gastric) the malignant potential of IPMNs varies significantly.
